Registration to
A short Course on NonLinear Mixed Models
and Their Applications

The registration deadline is 15th September. Please specify whether your registration includes the computer workshop, provide information on any dietary restrictions for the lunches, and whether meat, fish or vegetarian dish is preferred for the course dinner.


When registering, the following information need to be given:

1. Title (e.g. Mr/Ms/Mrs/Dr)
2. Firstname Surname
3. Are you attending the seminar+workshop or just the seminar
4. Which organisation(s) are you a member of (SSL/FSB/FMS/none)
5. Employer (Student/Company name)
6. Do you have any dietary restrictions
7. If you take part in the couse dinner, what is your dinner preference (meat/fish/vegetarian)



Payment

The registration fee should be payed to one of the following bank accounts.

- Participants from Finland should use SSL's bank account: 571113-225692 with Viitenro 30096
- International paticipants should use the following information:
     1. SSL's bank account number IBAN FI91 5711 1320 0256 92
     2. Account owner: Statisticians in Finnish Pharmaceutical Industry (SSL)
     3. Bank's SWIFT/BIC OKOYFIHH

     The SWIFT/BIC code itself includes the relevant information of the bank (the bank code/clearing number and adress).

Participants paying the early bird registration fee (290/210 EUR) should do it by 4th September.

NOTE! The workshop is now full but there are still seats available for the seminar. All participants who registered for the workshop before the early bird deadline have been accepted to the workshop. A confirmation letter will be sent on Monday 31.8. to all who have registered so far.

* The number of seats to the computer workshop is limited and therefore the Organizing Committee reserves the right to choose the participants for the workshop (first come, first serve as main principle). Participants paying the reduced student fee will be admitted if seats are available.

The registration fees (except for the reduced student fee) include coffee and lunch on all days, and a course dinner on the 30th September.

The reduced student fee is for full-time undergraduate students, i.e. not for postgraduate students or undergraduate students who work more than 70 hours a month.
